What is needed is an insurance company where everyone is included. The power of numbers allows the "SUPER AWESOME EVERYONE'S INCLUDED" health insurance company to negotiate great rates with PRIVATE hospitals, doctors, pharmacies, etc. for the best prices across the country. This will bring premiums per person down as the more members a company has the more negotiating power it has with PRIVATE medical providers. No one will be forced to join, the medical profession can continue to be the mix of private, public, and religious operations that it is now. Private providers will have a choice to take customers with SUPER AWESOME EVERYONE'S INCLUDED or not.
Of course I am describing single payer, or more precisely Medicare for Everybody but I think the term sucks. Some people are invested in being against it, some people have no idea what it means, "what, I have to singly pay for everything?", no matter how much you explain it to them. Re-branding and selling it like everyone is a 5 year old might be the trick. I think the name SUPER AWESOME EVERYONE"S INCLUDED is a joke, and medicare for everybody didn't work for some reason (I thought it was pretty good) so I am betting Daily Kos can make the sales pitch and name better than I can.
