The US using a fake polio campaign to spy on bin Laden's families DNA is only the last catalyst, of years of built up feelings of hurt anger and suspicion, Doctors Without Border's nurses offended Pashtun moral traditions 20 years ago by not even wearing a head scarf much less a burka and having a male relative escort which lead to deadly results. One should note that President Kennedy's wife Jackie when visiting Saudi Arabia wore a head scarf and Madeleine Albright and Hillary Clinton picked winter for a visit where they could be overdressed in a hooded coat while greeting Arab royalty. Many locals in Pakistan and Afghanistan are insulted that Americans don't respect them as much as they do the Saudis. I will note however that Afghan President Karzi did managed to do some things for woman’s rights. However he at first represented King Zahir Khan who many Afghans admire like many Americans long for the Kennedy years and Camelot, King Khan is one of the few Afghan governments not dominated by foreign countries.
The conflict over woman's rights in that part of the world and attracts against so-called uppity women both domestic and foreign actually goes back to 1921 when the Soviet Union under Lenin began pouring aid into Afghanistan,

For over two generations woman's rights and Communism meant the same thing in that part of the world as the Soviet Union poured in attempting to modernize the culture. In much of the third world Christianity for generations stood for shoving the local culture aside, in Afghanistan and parts of Pakistan it was Communism and its emphasis for equal employment and education for women that did so, which was often an excuse to discrediting local cultural values in unrelated ways.
So today there is a health care crisis in the rebellious tribal areas of Northern Pakistan that has no easy answers.
To me, I think the best starting solution would be a small group of volunteer trying to offer health care directly with the militants. Maybe with a lot of goodies included like night cameras. Women would be needed in order to work directly with mothers and their young children but they might have to have their husbands or other adult male relatives along and the tricky part appear someone subservient to their husbands and male relatives while wearing a burka.
One problem is that Saudi doctors and nurses while obviously have to see at times often covered parts of ones body, do so discretely and would be in no condition to go to Pakistan in any public way.
There are several possible hopeful scenarios but people tend to wait for the last minute in seeking solutions. I see only one last second scenario against disaster. Cuban doctor Felix Baez Sarria has no signs of Ebola scheduled to be officially cured December 3 has a doctor wife Dr. Vania Ferrer Santos who wanted to also serve in Africa but for the moment the Cuban contingent is all male. When the first rumors of Ebola enter tribal Pakistan he and his doctor wife could rush to fight Polio in the tribal area of Pakistan. He as an Ebola survivor needing no protection and she wearing an Ebola mask conveniently died black to look like a burka and her “space suit” also conveniently dark colored to blend in with the burkas surrounding her. Since a moment would come soon where she would have to choose, or not choose, to wear a real burka, and since the right-wing Cuban Americans hound whatever Cuba does including condemning Cuba for going to Africa (to risk in their mind bringing Ebola back to Cuba),

the US would have to at that moment quickly end the Embargo for her to risk the ridicule of putting on an actual burka. One side opportunity South Korean ebola medical workers or at least their government beg the media to keep their identities secret, and again Saudi doctors and nurses are very destreat in looking at parts of the body of those with the opposite sex. They could decreatly help the Cubans if the Ebola crisis gets out of hand in Pakistan. There are other possible scenarios but the above is the only last second possibility of stopping Ebola from potentially threatening Asia as much as Ebola threatens Africa. The blame game could lead to other problems particularly for the US. Possible even wounded US troops in Afghanistan no longer getting the warm human touch due to fear of Ebola and packages from the front from their relative on the front, mixed with a touch of fear of Ebola mixed with the joy of receiving a package from a loved one.
