This evening, much to my dislike, my warning was proved correct. His GFR is quite low, creatinine and BUN is high.bl
Glomerular filtration rate (GFR) measures how well the kidneys are functioning by estimating how much blood goes through the glomeruli (filters) in the kidney. BUN is blood urea nitrogen, creatinine is a breakdown product of creatinine phosphate from muscles. The BUN/creatinine ration gives a good idea how much function remains in the kidneys. The kidneys naturally filter out both BUN and creatinine, but they can also restrict passage if the body requires them (OK, I've really Goobered it down, but that's the general notion).
I don't know the precise numbers, as I do not personally know the man. But, in general terms, he might be ready to begin dialysis or will be fairly soon. The body doesn't like elevated glucose and hypertension, with glycated end products wreaking havoc on epithelial cells in the blood vessels.

Nearly always in an industrialized nation, universal, single payer health care exists. At least in civilized nations.
Of all of the industrialized nations in the world, only one not only fails to have single payer medical care, it doesn't have real universal health care.
Considering my wife suffered an injury in Dubai and received free treatment, that's really extending things a lot. Dubai isn't highly industrialized, their primary product is oil and being a transportation hub, as well as building really, really big buildings.
Hence, why I say that the United States of America isn't civilized and is trying its level best to achieve true third world nation status.
Back on topic, Mr Lang is in serious medical trouble, trouble that will keep him from productive work while going from specialist to specialist. I've traveled this path with my father, when I returned home from abroad to care for him.
It's not pleasant. Dialysis sucks as well.

He hasn't plead for more money, but having escorted my father through specialist after specialist, eventually to dialysis, eye treatments for wet macular degeneration and more, I know how damnably high those paltry copays pile up to.
As the liens on the property I've inherited can attest to.
So, I have a very good grasp of the pathophysiology of his condition, with his youth favoring partially in his favor. But, a condition long ignored causes long and short term pathology.
I do not envy his and his wife's path forward.
But, I recognize those who aren't entirely cognizant of their peril, due to ignorance of things medical.
